Please fill this form so we know what kind of Xbox you have.  If you have
more than one Xbox, fill this in for the Xbox with which you will be
testing (and only that Xbox!).


1. What is your Internet name and email address?  (Real name is optional)



2. What version of the retail Xbox kernel do you have?  To get to it, go to
   Settings then System Info.  The scrolling text will have a line like
   K:1.00.4034.1.  The K: is your kernel version.  If you don't see this
   line, you have version 3944.



3. What version of the Dashboard do you have?  It appears in the same place
   as the kernel's version with D: instead of K:.  Again, if it's not there,
   you have version 3944.



4. What motherboard revision is your Xbox (1.0, 1.1, 1.2, 1.3)?  If you
   don't know, write "don't know".



5. What region is your Xbox from?  (What is its region code?)



6. What video mode does your Xbox use?  (PAL-50, NTSC, HDTV, etc.)



7. What hardware upgrades have you done to your Xbox?  For example, mod
   chips, 128 megs of RAM, replaced hard drive.



8. Have you installed Linux on your Xbox before?



9. Are you able to recover from a worst-case scenario in this beta test, a
   dead clock with a corrupted/crashing Dashboard?  Please explain how you
   could recover.




By participating in this beta test, you acknowledge that there is risk in
using this program.  It is possible, due to a bug or otherwise, that if your
clock capacitor dies or some other setting becomes corrupted, you will be
unable to boot any disks and unable to load the Dashboard to fix your
settings.  Your Xbox will be useless unless you have some other means of
recovery, such as a mod chip or a Hardware Refresh Disk from Microsoft.
